\u203bAlthough the legal age of adulthood in Japan was lowered in April 2022, TCB adheres strictly to the following policy.

For Minors<\/h2>\r\n

To receive a consultation or treatment, a parent or legal guardian must accompany you and provide a signed consent form<\/span>.
Please download and print the consent form from the link below, complete it by hand, and bring it with you.
If you do not bring the completed consent form, we will not be able to offer you a consultation.<\/span> Thank you for your understanding.
In addition, the parent or guardian will be required to present a valid ID<\/span>. Please be sure to bring it when visiting the clinic.<\/p>\r\n\r\n

For Patients Aged 18 or 19<\/h2>\r\n

You do not need to provide a consent form, be accompanied by a parent or guardian, or undergo a phone confirmation in order to receive a consultation or treatment.
Additionally, in accordance with the Act on the Protection of Personal Information, we do not disclose patient information to a parent or guardian without the patient’s explicit consent.
