Fat injection is a highly popular procedure for areas such as the cheeks, under the eyes, nasolabial folds, lips, and forehead. It is not only effective in reducing wrinkles but also in restoring volume lost due to aging in areas like the cheeks and temples. Unlike hyaluronic acid injections, which are gradually absorbed by the body, fat injections use the patient’s own fat, and once established, the results can be semi-permanent. Furthermore, the latest fat grafting technology known as “Nano Fat 2.0” significantly reduces the risk of lumps, a concern with traditional fat grafting, while also improving fat retention — making the outcome safer and more natural.

What Is “Nano Fat 2.0”

Traditional fat grafting involves injecting harvested fat directly into the desired area. However, raw harvested fat may contain broken fibrous tissues and other impurities that can hinder retention and may even lead to lump formation. To address this, techniques such as “Condensed Rich Fat (CRF)” have been developed, which concentrate the fat and remove impurities before injection. “Nano Fat 2.0” goes a step further by further enhancing the retention rate while minimizing the risk of lumps. This makes it a safer and more advanced option for natural-looking facial rejuvenation.

Preparation and Anesthesia Before Fat Harvesting

■ Preparation and Anesthesia Before Fat Harvesting: Make a ~5mm incision at the cannula entry site, avoiding the femoral artery and vein, and administer local anesthesia.
The injection consists of a tumescent solution：125ml (100ml saline + 25ml 1% epinephrine).
For injections under the eyes or into the nasojugal fold (tear trough), approximately 50–60cc of tumescent solution is usually sufficient.
In lean patients requiring fat harvesting from broader areas—or when injecting into the forehead, temples, or cheeks—the entire prepared anesthetic volume may be used, or harvesting may be performed bilaterally.
Avoid bolus (high-volume) injection of anesthetic in a localized area, as it makes fat harvesting more difficult and painful for the patient. Distribute the anesthetic evenly throughout the area.

■ Estimating Harvest and Injection Volume: To obtain about 6cc of usable fat post-centrifugation, aim to harvest around 15–20cc.
(Centrifugation typically reduces fat volume by more than half.)
Aspirate from the middle to deep fat layers. The orientation of the cannula port should generally face downward.

Anesthesia and Injection Site Preparation

■ Anesthesia and Injection Site Preparation: Local anesthesia is administered to the planned microcannula entry points. Additionally, infraorbital nerve block is performed.
Using an 18G needle, create 2–3 entry sites per side, inserting them in a relatively vertical direction.
Fat is injected in a retrograde fashion while changing layers, avoiding bolus (bulk) injection.

How many cc are injected per treatment area?: It depends on the area. For example, 0.1–0.3cc is typically used for tear troughs, 0.6–1cc for wrinkles, and more than 1cc for volume loss.

Can the injected fat be removed?: Unlike hyaluronic acid, fat cannot be dissolved or reversed. If removal is necessary, multiple sessions of fat-dissolving injections (e.g., BNLS) may be required. Generally, it is difficult to fully reverse the results.

How many sessions are needed for visible results?: Many patients see noticeable improvement after just one session. However, depending on the area and desired volume, multiple sessions may be needed.

Can fat injections improve skin texture?: Yes. Especially with Nano-Fat injections, the stem cell-like components and growth factors may enhance skin elasticity, tone, and radiance.

Can I receive additional injections soon after the first procedure?: It is not recommended to inject the same area again in a short period due to fat retention considerations. However, it is possible to treat different areas within a short timeframe.
